A select group of Security Health Plan members with a congestive heart failure (CHF) diagnosis will be invited to join a new Medication Care Management® program offered at no cost by Curant Health on behalf of Security Health Plan.
Security Health Plan and Curant Health, a national patient support organization, have identified nearly 600 of the Plan’s Medicare Advantage members to receive an initial letter explaining the Medication Care Management® program’s benefits. These members will then receive a phone call from a Curant Health clinical assistant to initiate the program. Program benefits include:
- Assignment of a dedicated nurse care manager to support the member’s specific needs and provide guidance.
- Support from a clinical team of nurses, pharmacists, medical assistants and pharmacy technicians to help the member manage medications and their side effects as well as answer health-related concerns.
- Specific, relevant health and medication information provided through a secure website.
“Through our partnership with Curant Health, we are giving our members with CHF the tools and support they need to actively manage their health,” said Heather Kurtz, director of population health at Security Health Plan. “We’re confident that support from Curant Health will enrich our members’ lives and help them feel their best every day.”
Michell Butler, PharmD, senior director of clinical services at Curant Health, said her team will provide Security Health Plan members with education, preventive care and personalized health management tools that will improve health and assure the effectiveness of prescribed medications.
